1、 Fill in

1. Experimental commission form 1 requires clear and neat handwriting. For example, the number 0 and the English alphabet O, the number 2 and the English alphabet Z should be clearly marked;

2. Fill out each item of the commission form in detail according to your needs to ensure the accuracy of the information;

3. Select report format: If a Chinese report is required, please fill it out in Chinese; English report required, please fill in both Chinese and English; Items such as item number to destination country can be selected according to customer requirements and are not mandatory.

4. Please indicate the contact information of the reporting unit in the remarks column when selecting the post sample inspection processing method and report delivery method for more accurate reporting;

5. If the report needs to indicate other information or order number or batch number, please provide an explanation;

6. Service requirements for selection: Normal work cycle of 5-7 working days, available for expedited service;

7. Signature or official seal of the agent of the entrusting party.

2、 Handling Testing by Postal Sample Delivery

1. The customer will fill out a clear commission form and deliver the sample to our center together, with the courier fee to be borne by themselves;

2. After receiving the sample and commission form, both parties agree on the technical plan and cost of this test, and arrange for testing. Our center will send the fee notice to the customer by fax. The customer will arrange payment as soon as possible before the report is issued, and fax a copy of the payment voucher to our center for reconciliation;

3. After receiving the accurate payment voucher by fax, fax the report to the customer on time;

4. The original report and invoice (to be issued after financial confirmation) shall be collected or sent by courier according to the customer's requirements.
